敬告

1、申請人在填寫本申請書前，可登陸深圳市市場監督管理局網站（http://www.szscjg.gov.cn/）參考企業注冊分類指南和各類示範文本，也可到各注冊大廳領取。

2、申請人應當使用黑色鋼筆、簽字筆或毛筆工整填寫有關內容和簽名，除簽名須由本人簽署外，其他內容可以打印。

3、申請人提交的文件、證件應當規範、清晰、潔淨，應當使用A4紙。

深圳市市場監督管理局：

根據法律、法規等相關規定，現申請企業登記，請予核準。同時承諾：所提交的文件、證件和有關附件真實、合法、有效，複印文本與原件一致，並對因提交虛假文件、證件所引發的一切後果承擔相應的法律責任。

Cautions:

1.Before completing this application form,the applicants can refer to the classification guides about enterprises register and all kinds of demonstrative texts from the website of Shenzhen Market Supervisory Authority(http://www.szscjg.gov.cn/).They also can obtain the reference from the registration hall.

2.The applicants shall use a black pen,sign pen or brush to fill in related contents and sign their names neatly. The application form can be printed except the signatures.

3.The submitted documents and certificates should be standardized, clear, clean,and in the paper form of A4.

According to laws, regulations and other relevant provisions, we apply for registration of the enterprise now, and we hope you can approve it.
